Notice to Patients: Insurance & Private-Pay Election
Qyana Griffith, MD, PC provides specialized ExoMind™ Neuromodulation protocols that are classified as “off-label” and do not meet the criteria for standard insurance coverage or Medicare reimbursement. By electing these services, patients acknowledge they are entering into a private-pay agreement and waive the right to submit these claims to third-party payers for CPT codes 90867 or 90868. For a detailed Good Faith Estimate of costs for your specific protocol, please review your Right to a Good Faith Estimate or contact our office directly pursuant to the No Surprises Act.
Out-of-network reimbursement
Ask the office whether documentation may be available for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on your individual plan.

Patients who are uninsured or not using insurance may request a Good Faith Estimate of expected charges before scheduling a service.
